1. Collect the empty wrappers/labels of the following participating brands

a. Nescafe 3in1 Original/Brown N’Creamy/Chocolate/Creamylatte (2 empty wrappers)

b. Milo Chocolate Powdered Drink  Milo 22g or Milo R2 24g (2 empty wrappers or any combination)

c. Family’s Brand Sardines One (1) label of Family’s Brand Sardines of any variant

d. Smart Prepaid One (1) entry for every Php15 worth of Smart load

e. CDO One (1) can label of CDO Meal loaf or CDO Beef Loaf of any size

f. Aji-Ginisa Flavor Seasoning Mix Six (6) empty packs of 7g Aji-Ginisa Original

g. Energen Two (2) empty sachets of Energen Cereal and Energen Go-Fruit or any combination

*Combination of proof-of-purchase from different sponsors is not allowed.
*Tampered and damaged entries shall invalidate the entry. 70% of the proof of purchase shall be kept intact.

2. How to send entries:

a. For Nescafe 3n1, Milo Chocolate Powdered Drink, Family’s Brand Sardines, CDO, Aji-Ginisa, and Energen

i. Enclose in an envelope the proof-of-purchase in a legal-sized envelope and write the following at the back of the envelope.

• Enclosed Participating Brand
• Area (NCR, Luzon, Visayas, Mindanao)

ii.  In a separate sheet of paper, write your name, age, address, contact number and signature.
*Hand-written/printed out entry forms are valid, as long as signature is handwritten
iii. Drop in the drop boxes located in select Mercury Drugstore, GMA Regional TV & Radio Stations, or at GMA Network Center, EDSA corner Timog Avenue, Quezon City

b. For Smart Prepaid
i. Prepaid subscribers have to register to Kapuso Milyonaryo by texting the following syntax to 4627:
KM<space>REG<space>Name, Complete Home Address, Area
Example: KM REG Juan Dela Cruz, 123 Zapote St., Las Pinas City, NCR
*Registration is required once, and is free of charge.
ii. Every Php15.00 load purchased electronically from the date of registration will be automatically converted to one (1) entry.

3. How to win

a. Weekly Prizes:
